FC is better at inflammation than diagnosis
Calprotectin is released by neutrophils. A low value makes substantial active intestinal inflammation less likely; a high value says inflammation is plausible but does not identify the cause. It should therefore be used to answer a probability question, not to declare 'Crohn's disease' from a stool result.
Diagnosis versus IBS
In a patient with chronic diarrhea and no alarm features, FC can help determine whether an inflammatory work-up is justified. The 2025 Crohn's guideline notes a practical cut-off range around 50-100 micrograms/g for distinguishing inflammatory from non-inflammatory disease, but laboratory assays and pretest probability matter.
Monitoring treatment response
Falling FC after induction supports biologic response and can reduce reliance on repeated endoscopy in the short term. Persistent elevation despite symptom improvement raises concern for ongoing mucosal inflammation and should prompt reassessment rather than automatic reassurance.
Suspected relapse
In a patient with known IBD who develops diarrhea or urgency, FC can help distinguish inflammatory relapse from IBS overlap, bile acid diarrhea, infection or another non-inflammatory cause. It should often be paired with stool infection testing when clinically indicated.
Do not overreact to a single borderline result
Day-to-day biological variability and assay differences mean borderline values are often best repeated in context. Trends can be more informative than isolated numbers, especially in an otherwise stable patient.
A practical clinical approach
- Define whether the question is diagnosis, response, maintenance monitoring or suspected relapse.
- Interpret FC alongside symptoms, infection testing and CRP rather than in isolation.
- Use low results to avoid unnecessary invasive testing when pretest risk is low.
- Use persistently high or rising results to trigger objective reassessment.
- When values are borderline and the patient is stable, repeat before making a major treatment change.
Common errors to avoid
- Calling any elevated FC 'IBD'.
- Escalating advanced therapy from a single borderline result.
- Ignoring enteric infection.
- Assuming a low FC completely excludes isolated small-bowel Crohn's disease.
What should trainees remember?
Fecal calprotectin is a probability and monitoring tool. Its greatest value is reducing the gap between symptoms and objective inflammatory activity.
